Frequently Asked Questions About Birth Preparation Physiotherapy
1. When should I begin birth preparation physiotherapy?
Ideally, you can start after the first trimester. This is because internal pelvic floor assessments are safest from the second trimester onwards, once pregnancy is more stable. However, if you’d like to start earlier, prenatal physiotherapy sessions can still be helpful in the first trimester. These sessions focus on gentle exercises, posture support, and breathing techniques to help your body adapt as your pregnancy progresses.
2. What does a pelvic floor examination involve?
An internal vaginal assessment is considered the most accurate way to evaluate pelvic floor strength, coordination, and flexibility. If you’re not comfortable with an internal exam or have medical restrictions, alternatives include:
-
Transperineal ultrasound assessment — a non-invasive external scan
-
External vaginal assessment — gentle palpation from outside the vaginal opening

Your preferences and comfort are always prioritised, and the assessment method will be tailored to your needs.
3. How is birth preparation physiotherapy different from seeing an OBGYN?
An OBGYN focuses on monitoring your pregnancy, managing potential risks, and ensuring medical safety during labour and delivery.
Birth preparation physiotherapy, on the other hand, focuses on preparing your body and pelvic floor for birth and recovery. Together, these two approaches provide a comprehensive support system that addresses your medical, physical, and emotional well-being throughout pregnancy, delivery, and postpartum recovery.
4. What types of pains are common during pregnancy?
Pregnancy brings about hormonal, physical, and postural changes that can make muscles, ligaments, and joints more sensitive. Some common areas where women experience discomfort include:
-
Lower back and pelvic girdle
-
Hips and ribs
-
Neck and shoulders
-
Wrists and hands

While these pains are common, they are not always normal and shouldn’t be ignored. A women’s health physiotherapist can help identify the cause and guide you through safe strategies to relieve pain and support your body for birth.
5. Is birth preparation physiotherapy only for mums planning a vaginal delivery?
Not at all. Birth preparation physiotherapy benefits all mums, regardless of whether you’re planning a vaginal birth or a caesarean section. Sessions focus on optimising pelvic health, core strength, and overall mobility, which supports a smoother birth and promotes faster recovery after delivery. No matter the mode of birth.
6. What is a perineal massage and how does it help?
Perineal massage gently stretches and prepares the tissues around the vaginal opening for childbirth. It helps improve flexibility and reduces the risk of severe tears or the need for an episiotomy during vaginal delivery. Evidence shows that starting perineal massage in the later weeks of pregnancy can support a more comfortable birthing experience.
